1) Please click through the pop-up promo in mafia wars to initiate the promo in poker.
2) You will see a popup in Zynga Poker indicating the promo has begun.
3) Once you gain three levels, you will see a popup in Poker.
4) Please then click the green button in this popup in order to get the click through effect and be greeted in Mafia Wars with the congratulations popup, and be awarded the item.
5) If you did not click the accept button, please use the following link. If you have gained the 3 Poker levels, this will award you the item and the achievement.

Note: for the first time Zynga Poker player you need to reach at least level 6 in Zynga Poker to get Pocket Rockets item and achievement, plus you will also get Ace In Sleeve item and achievement if you didn't get it yet.

@Jodi --> Please check in your facebook application setting if you have disabled Texas HoldEm Poker. I had the same problem.
Didn't work for me. On level 6 now and haven't got either the pocket rocket, ace in the sleeve or the achievments. :(
